Release & Standard Terms (December Program)

IMPORTANT NOTICE: This is an important document and you should read it carefully before agreeing to it. Upon accepting this Release and Standard Terms, you acknowledge that you have read and understood these terms and conditions.

1. General Waivers, Releases & Acknowledgments:

In consideration of “Heyfootball Pte Ltd”, being the relevant training division of F17 Football Academy (“F17”), allowing the Participant membership of and participation in the practicing or playing of soccer, managed or instructed by F17 or any Representative including any related social activities at any location whatsoever (“F17 December Program”), the Participant acknowledges and agrees:

(i) to waive any and all claims that the Participant has or may have against F17 or any Representative, as either a direct or indirect result of, the Participant’s membership or participation in F17 December Program;

(ii) to release, hold harmless and indemnify F17 and each Representative from any and all liability for any loss, damage, injury or expense that a Participant (including his/her immediate or extended family) may suffer or incur, as a direct or indirect result of the Participant’s membership or participation in the F17 December Program due to any cause whatsoever, including but not limited to negligence, breach of contract, breach of any statutory or other duty of care, including but not limited to any duty of care on the part of F17 or any Representative and

(iii) that the F17 December Program are potentially hazardous activities and agrees that the Participant is voluntarily entering and assuming all risks associated with participating in F17 December Program including but not limited to falls, bodily contact with other participants, the effects of weather including but not limited to lightning, heat or humidity, and the conditions of the training grounds and facilities used for the conduct of the F17 December Program, and acknowledges that it understands and appreciates all such risks.

2. Medical Waivers, Releases & Acknowledgments:

In consideration of F17 allowing the Participant membership of and participation in the F17 December Program, the Participant acknowledges that he/she has been advised and is aware that:

(i) neither F17 nor any Representative has any medical training that would provide assistance to an injured or ill Participant, nor do they possess or maintain any medical equipment or apparatus (including but not limited to defibrillators or epi-pens) which may assist an injured or ill Participant;

(ii) if F17 or Representative is provided with any medical equipment or apparatus for a Participant, by a Participant or any third party, F17 or Participant makes no representation or warranty that it has the skill or training to operate such medical equipment or apparatus appropriately to assist any participant or to prevent further illness or injury to a Participant and

(iii) he/ she is solely responsible to select and purchase adequate medical/health and travel insurance for the Participant and all other risks whatsoever, and that no medical/health or travel insurance will be provided by F17 or a Representative, and no F17 or Representative accepts any responsibility for any costs or damages associated with a medical/health or travel related problem, nor will F17 or Representative pay for any medical/health or travel related expenses, which may be incurred by a Participant during the F17 December Program.

3. Photography & Media:

In consideration of each of F17 allowing the Participant membership of and participation in the F17 December Program, the Participant authorizes F17 to allow the reproduction, dissemination, and/or publication of the name of the Participant and/or likeness (including any minor child who is a Participant) for media coverage, public relations, advertising, marketing or any other F17’s promotional or educational purposes. This may be done in conjunction with the Participant’s participation in any F17 December Program, and the Participant acknowledges and agrees that it, or any minor child who is a Participant, will not be entitled to receive any fee or compensation for such use. The Participant hereby authorizes the processing of the above personal information in order for his/her registration for the F17 December Program.

4. Cancellations and No-Shows:

If the Participant elects to cancel any reservation, F17 must receive written notification of such cancellation via electronic mail to info@f-17.org (or such other email address as notified by F17 from time to time).

The Participant acknowledges that:

(i) if the Participant does not present himself/herself at the F17 December Program start date and duration of the F17 December Program, there will be no refunds of any amounts paid by the Participant;

(ii) in the event of heavy rainfall or lightning or any related event that will make the conduct and completion of the F17 December Program impossible (as decided by F17 in their absolute discretion), F17 reserves the right to cancel any or all F17 December Program and in such circumstances, the Participant shall not be entitled to any refund whatsoever, and no replacement or alternative activities will be organized.

5. Defined Terms:

In this Release & Standard Terms, “Participant” means the person who’s name and identification details are set out above, including, where relevant, the above mentioned minor child.

“Representative” means any affiliate, officer, director, shareholder, agent, contractor, employee, instructor, trainer, volunteer, member or any other representative of F17.
